  • List three documented benefits of pharmacist-managed anticoagulation services.
  • Describe the role of the pharmacist in preventing and managing bleeding and other potential problems associated with anticoagulant therapy.
  • Identify a common reason for clinician nonadherence to guidelines for the use of anticoagulant or antiplatelet therapy in patients with ACS or patients with or at risk for VTE.
  • Develop a plan to improve adherence in patients with ACS or patients with or at risk for VTE.
  • Identify a common reason for nonadherence to anticoagulant or antiplatelet therapy in patient with ACS or a patient with or at risk for VTE and devise a strategy to overcome the problem.
